Required Forms for Elementary Schools to Certify Sufficiency of Instructional Materials to Board of Education

As outlined in Site Operations Bulletin No. 3

To certify sufficiency, these forms must be completed by all elementary schools not subject to a Williams visit.

Attachment A- Instructional Materials Sufficiency Survey for Grades UTK-5 - this document to be completed based on the Destiny textbook inventory and physical inventories of adopted student instructional materials and PowerSchool enrollments of approximately August 16, 2024. 

Attachment A- Biliteracy Instructional Materials Sufficiency Survey for Grades UTK-5 - this document is to be completed in lieu of Attachment A by the staff of school sites with a biliteracy program. The form is used to identify their inventory and ensure sufficient quantities of instructional materials are available in English and/or Spanish to support their students. In order to fill in the form, click on the link above, open the Word document, select the View Tab at the top left corner of the document, and then select Edit.

Attachment B-Principal Verification Statement-Instructional Materials Sufficiency and UCP Posting for Elementary Schools -this form is to be completed by each Principal no later than August 30, 2024 to verify they have sufficient core student instructional materials for their students and the Uniform Complaint Procedure is visibly posted in each classroom. This completed, signed, and dated form must be returned to the Instructional Materials Office by emailing to [email protected]. In order to fill in the form, click on the link above, open the Word document, select the View Tab at the top left corner of the document, and then select Edit Document.

Uniform Complaint Classroom Postings to be displayed in each classroom and Uniform Complaint Forms must be available for parents wishing to file a complaint relating to Williams legislation.  Access these forms on our Uniform Complaint Procedure Menu Tab.
