BEFORE YOU COMPLETE THE APPROPRIATE INTAKE FORM, SIMULTANEOUSLY ENSURE THE CONTRACTOR/VENDOR HAS A VENDOR # IN PEOPLESOFT. THAT CAN BE DONE THROUGH THIS SIMPLE REQUEST.
1. Independent Contractor Agreements (ICAs) - ASB FUNDS ONLY (Not to Exceed $10,000.00)
NOTE: PLEASE DO NOT USE ICA Forms FOR VENUE AGREEMENTS (Utilize the GSA Intake Form)

Do this First: For all services provided by an "Independent Contractor" (they cannot have a License Issued by the State of California) please contact Human Resources FIRST by completing this questionnaire. Wait for a response from H. R. before you complete any ICA forms listed below.

- Use this intake form for Non-Students Instruction Services (No License issued by the State of California Required) such as Independent Contractors for Dance, Music, Pottery, Gardening, Acting, Venue Agreements, ASB Venues included such as for Proms, Graduations...etc. Please contact Human Resources FIRST by completing this questionnaire.
