CONTRACT INTAKE FORMS

BELOW ARE THE VARIOUS TYPES OF CONTRACTING METHODS AVAILABLE TO SCHOOL SITES AND DEPARTMENTS.

BEFORE YOU COMPLETE THE APPROPRIATE INTAKE FORM, SIMULTANEOUSLY ENSURE THE CONTRACTOR/VENDOR HAS A VENDOR # IN PEOPLESOFT. THAT CAN BE DONE THROUGH THIS SIMPLE REQUEST.

1. ASB Funds Only- Independent Contractor Agreement Form

  • PLEASE DO NOT USE FOR VENUE AGREEMENTS (Utilize the GSA Intake Form)

  • Complete the DocuSign ICA PowerForm first

  • To complete the approval process, download the completed PowerForm and submit it with an Intake Form  

  • School Sites must follow ASB procedures and policies to include requesting the Insurance Certificate from the vendor/service provider.

2. Stipends and Fee Reimbursements Intake Form

3. No Cost Partnership Services Intake Form

  • The organization has an Agreement to sign

  • There is no cost to participate

  • The District is not obligated to provide data or information to the organization

4. General Service, Venue, Public Agency, and Private School Agreement Intake Form

  • For all Student Instructional Services by a Professional that has a license issued by the State of California (i.e. Teachers, Attorney's, Doctors, Nurses, Phycologists,  Mental Health Services, Police, Fire, Clinicians, etc...) please contact HR (Human Resources) FIRST at [email protected] and complete their questionnaire.

  • For Non-Students Instruction Services (No License issued by the State of California Required. i.e. Independent Contractors for Dance, Music, Pottery, Gardening, Acting...etc), Please complete EXHIBIT B, before completing the GSA Intake.

  • Utilize this Intake Form for all Venue Agreements (ASB Venues included) such as for Proms, Graduations, etc....

  • All funding types can be utilized under a GSA.

  • Less than $109,300 in expenses in a calendar year.

5. Professional Services, Software License Agreements, and Piggybacks

  • All funding types can be utilized.

  • Professional Services (Requires a License issued by the State of California, i.e. Teachers, Attorney's, Doctors, Nurses, Phycologists,  Clinicians, Real Estate Agents, Insurance Brokers, Architects, CPA's, Financial Advisors, etc...)

  • Scope of work (SOW) must be submitted in Word Format (Arial Narrow 11 Font, and single spaced) Here is the link to the sample SOW: Sample SOW

  • Intake forms without a Scope of Work and no IRS Form W9 will be rejected.

6. Invitation to Bid (ITB) for goods and products, and Request for Formal Proposal (RFP) for services

7. Maintenance and Operations

  • Routine Maintenance and Repair Services for Physical Plant Operations, grounds, landscaping, and custodial

  • Less than $99,100 for the term of the Agreement

8. PPO and Facility, Planning, and Construction

  • Projects greater than $15K

  • Installation or alteration of a public building

9. FPC - Professional Services

  • Facility projects that require professional services

  • IT services supporting facility projects, paid with Bond funds
