The San Diego Unified School District provides leave due to certain circumstances related to employees impacted by COVID-19. This leave is in compliance with both SB95 and the applicable negotiated agreement. Similar leaves were previous authorized under the Families First Coronavirus Response Act (FFCRA), Public Law 116-127,  which was effective April 1, 2020 through December 31, 2020. 
The EPSLA allows eligible employees to request two (2) workweeks (up to 80 hours) of paid sick leave for specific reasons.  

To use this leave, obtain the applicable physician's statement and complete the form below:         

These forms are maintained by your site timekeeper; you do not need to turn the form in to the HR Division.
