The Public Works (Construction) "Professional Services" Contract Request Form was developed to assist FPC & PPO with the creation of a contract between the district and a professional service provider in support of a construction (public works) project.

The detailed information provided within the form is critical to expedite the creation of that agreement. Please complete all fields to the best of your ability.

Once started, you must complete the form. So please have all the required information readily available.
