Contract Intake Form for Public Works

The Public Works Intake Form was developed to assist the FPC and PPO departments with the creation of a contract between the district and contractor.

The information contained within the form is critical to expedite the creation of that Agreement. Please complete all fields to the best of your ability. You may not start and stop the form.

Things to Consider when Contracting for a Public Works Project:

Determine whether the service is a true public project or is considered maintenance work.

Determine whether CEQA is needed and has been filed.

Determine whether AOR specifications are complete and up-to-date, and are ready for the ITB.

Engage multiple Executive Director Stakeholders in this procurement therefore allowing the opportunity to procure like services at multiple sites in order to expedite the work.

Definitions

Public Project means:

(1) Construction, reconstruction, erection, alteration, renovation, improvement,

demolition, and repair work involving any publicly owned, leased, or operated

facility.

(2) Painting or repainting of any publicly owned, leased, or operated facility.

"Public project" does not include maintenance work.

"Facility" means any plant, building, structure, ground facility, or utility system.

Public projects must be bid-out as a formal competitive bid. This process could take from 8-24 weeks depending on the stage of the procurement.

Maintenance Work means:

(1) Routine, recurring, and usual work for the preservation or protection of any publicly owned or operated facility for its intended purposes.

(2) Minor repainting.

(3) Resurfacing of streets and highways at less than one inch.

(4) Landscape maintenance, including mowing, watering, trimming, pruning, planting, replacement of plants, and servicing of irrigation and sprinkler systems.
