Contract Intake Form Maintenance and Operations

The Maintenance & Operations (M&O) Service Intake Form was developed to assist the Physical Plant Operations (PPO) department with the creation of a contract between the district and contractor.

The information contained within the form is critical to expedite the creation of that Agreement. Please complete all fields to the best of your ability. You may not start and stop the form.

Things to Consider when Contracting for Routine Maintenance Services

Determine whether the service is a public project or maintenance work.

Ensure you have sourced two quotes for services greater than $10K.

Request insurance certificates from your chosen contractor when you request a quote.

Ensure the total annual value of the routine maintenance does not exceed $95,200.

If the annual service work is greater than $95,200, a formal bid must be completed.

Definitions

Public Project means:

(1) Construction, reconstruction, erection, alteration, renovation, improvement,

demolition, and repair work involving any publicly owned, leased, or operated

facility.

(2) Painting or repainting of any publicly owned, leased, or operated facility.

"Public project" does not include maintenance work.

"Facility" means any plant, building, structure, ground facility, or utility system.

Public projects must be bid-out as a formal competitive bid. This process could take from 8-24 weeks depending on the stage of the procurement.

Maintenance Work means:

(1) Routine, recurring, and usual work for the preservation or protection of any publicly owned or operated facility for its intended purposes.

(2) Minor repainting.

(3) Resurfacing of streets and highways at less than one inch.

(4) Landscape maintenance, including mowing, watering, trimming, pruning, planting, replacement of plants, and servicing of irrigation and sprinkler systems.
